Important Note: In order to avoid any surprises regarding in network status, please send a picture of the front and back of your insurance card and indicate your Date of Birth. Your D.O.B is necessary for insurance verification purposes. We will have to verify insurance prior to scheduling our intake session.

As a general note it is the client's responsibility to know their mental health benefits. The phone number (typically listed as 'member services') on the back of your insurance card will link you with your plan directly. It is important to know your payment responsibilities including copays, co-insurance and deductibles. Payment for services will be collected at the end of each session.
I encourage you to contact your insurance provider before securing services to discuss your benefits.
Out of Network
Out of Network
Clients may choose to utilize their out-of-network benefits if their plan allows this. A 'courtesy' claim will be submitted to your insurance company on your behalf. Your insurance may, if applicable, reimburse a portion of your payment to you directly or they may apply that amount to your deductible.
It is important to understand that an out of network deductible may be separate from an in network deductible. Please seek clarity from your insurance provider directly as they determine & control all of this.

If you do not have insurance or you choose not to utilize your insurance benefits the following rates apply:
-Initial Assessment/Intake (60 minutes): $185
-Follow-Up Psychotherapy/Individual sessions (60 minutes): $180
-Follow-Up Psychotherapy/Individual sessions (45 minutes): $160
All payments are due following each session. In addition, see the Good Faith Estimate below.

Lynette Cisneros, PLLC requires at least a 24 hour notice to cancel/reschedule an appointment. Cancellations made less that 24 hours before a scheduled session will result in a $75 fee charged to the credit card on file for the missed session.
